                                  Enrollment Instructions 


              Currently enrolling for the 2025-2026 school year.

Returning Families             

(1) All returning families must submit the new Google Forms, "Returning Family Application", which includes a place to log the previous year's attendance. You will not need to resubmit the documents that you submitted upon your initial enrollment. This form will be completed on-line.
(2) Mail, paypal, or pay enrollment fees on-line as listed on the enrollment form.  

                                            

                                                                 Due by July 31, to avoid late fees.

New Families          **Year-Round Open Enrollment  ***

(1)  If you are a new family, you must have approval from Administrator before applying.  Please call or text number below (this simply alerts us to check on your application).  Utilize the above application to submit the online application.
UPLOAD THE FOLLOWING DOCUMENTS USING GREEN BUTTON ABOVE:

(a) Submit School Entry Health Exam completed by your child's doctor (Click here for form: DH 3040). -- It must contain current                         information  (no more than one year old).  This form is submitted only upon initial enrollment, {per FL1003.22}.

(b) Submit Immunization Record (Form DH 680 OR DH 681 see below) --  DH 680 must be current (no more than one                     year old).  This form is submitted upon enrollment and is to be updated when your child enrolls in the 7th grade,                     according to the Florida Statutes.

 ****Note: If your religious convictions conflict with requirements:
        You may obtain Form DH 681 issued by the Health Department which states that you have religious convictions to                 vaccinations.   Simply go to the local Health Department, during their immunization hours, and ask for a Form DH 681.          You will need one form for EACH child.

(c) Submit a copy of your child's birth certificate. 
 
(*) If you don't want to upload, you may mail the documents and mail or paypal enrollment fees.  If you are unsure about the fees needed, please contact the Administrator.  


                                           


When all of this documentation is received, your student will be enrolled.

This includes the establishment of your transcript at Risen Star Academy and includes either 9th grade alone (if you choose) or 9th and 10th grade. Each additional year will be submitted will be $40/ year.
